Nuke
Nuke was a guinea pig for Project Weapon Plus, the same military program that gave us Captain America. Unfortunately, an upper hand driven within him made him lose his mind and he became a ruthless weapon causing bloodshed wherever he stomped. Originally hired by Kingpin to kill Daredevil, he went berserk and was only stopped when Captain America and Daredevil apparently killed him.
Mr. Fear
Mr. Fear first appeared in Daredevil #6 in 1965.
